Librarian Wellness Survey

Dear Librarians— We are hospital librarians and we received a small grant from the Mid Atlantic Chapter of the Medical Library Association to conduct a research survey looking at the wellness behaviors of librarians. Our research proposal has been reviewed and approved by the Children’s National Institutional Review Board as an expedited protocol. The survey will take less than 5 minutes to complete. It is completely voluntary and confidential. If you would like to either receive a small thank-you gift ($10 gift card from Target–limited to approximately 187 respondents) and/or receive updates about this project, please complete the final form.
